not having looked into your cfg, can you check this two things:
- the format of your "log_prefix" parameter, for reference compare to
e.g. this one "log_prefix="{$mt $hdr(CSeq) $ci} "
- do you have a a request_route configured?

The last error is repeated many times (each time a remote user tries
to connect I think).
I can send any additional files if needed.
