is it somehow possible to bind to a variable ip-address and port?
e.g. getting the ip and port from a database?

You can execute your sql command and pass the result to kamailio in the
start command, I.e.:
Kamailio -f kamailio.cfg -A “BINDADDRESS=192.168.1.1:5060”
And in the cfg:
listen=BINDADDRESS
