You just need to t_relay() for stateful forwarding or forward() for stateless forwarding, but before you do you need to rewrite the R-URI host or port(optionally). You can use any of these functions according to you requirement and setup

rewritehost() / sethost() OR rewritehostport() / sethostport() OR rewriteuri(str) / seturi(str)
