If you're going to recommend to upgrade/or use something else - this is not feasible for some weird reasons. Is this a bug or just my misunderstanding how to use this function ?

upgrading will not help. The best is trying to avoid using the two operations over the same message. Alternative is to use msg_apply_changes() in between them.
It is due to internal architecture of how changes are done to the sip message. They are not applied immediately -- see more details here:

after messing around with msg_apply_changes() I gave up because it screwed up my Record-Route: so I ended up with rtpproxy_manage("...", "IP ADDRESS") which temporarily solved my problem. For now I'm facing firewall issues but this is completely different story.

Apparently 2nd rule doesn't work rewrite, it's doing append instead. What I want to achieve is : if SDP part contains BOT_RTP_ADDR -> rewrite it. In case SDP part contains RTP_INT_ADDR-> rewrite it, so 2x if.
I have tried to use sdpops module, it's coring in my case and I have a reason to not upgrade, I just can't. Any other ideas how to handle my problem ?
