We have three different OpenSER systems all pointed to a single MySQL database. It may be too late to ask now, but is that supported? Only one OpenSER system should be accepting registrations from a single phone at a time, and therefore only one OpenSER system should be updating the location table for a specific phone at a time. Correct?

do you have any previous or next related err messaged - it's important to find out the module that generates the query and the context.

Doug,
there error is from usrloc module - a contact for same user is inserted twice. I guess you are using cached usrloc. Even if in theory what you are saying seams right, the practice shows something else - more than one servers trying to add the same contact. Check you configuration and do some network trace to see where the problem is.
