Hve anyone seen before the following error?
*Kamailio acc [acc.c:398]: failed to insert into database*
After a Kamailio restart it is ok but it start again afetr x time.

what version are you using? Is mysql restarted? Any chance to reproduce it with higher debug level?

got your log messages, can you try with the latest git version for branch 3.1? You said you had kamailio 3.1.0, since that release there were some fixes with some modules that opened the database connection improperly, resulting in sharing same connection among many worker processes.
