One issue I've noticed: If you have an empty comment line (just # on a single line), then the next line is wrongly highlighted as comments.
For example:
# loadmodule "db_postgres.so"

Fixed and published a new version of the extension, since I took the time to add detection of indexes and transformations for pseudo-variables:
