I do see it in the portage tree but what about the ser-mysql integration and all the other add-ons. I don't see them in the portage tree. Do I need them or does ser come with them already included in the package that comes from the portage tree distribution.

The packages has a "mysql" USE flag, but I'm not sure if this is all you need. sipsak is also in the portage tree.

If you set the "mysql" USE flag, the mysql module will be compiled. But you still have to configure it (mysql is not activated in the default configuration). I dont know what you mean by other add-ons? RTP-proxy? That is not in portage yet.
