I need to change UA' ip address in Via header in Register request before forwarded to my network? Does the topoh module used in this situation? an use case for the topoh module is the topology hiding of forwarded messages (in most cases from INVITE transactions) to your downstream customers or provider.
Therefore this module will skip REGISTER (and also PUBLISH) messages. I think you can't use this for this requirement. You can of course always just use the textops module to modify the VIA, but this is not recommended.
If you really like to hide the VIA from REGISTER requests, considering using a B2BUA like SEMS or the the one from frafros: https://www.frafos.com/upload/html/26__Topology_Hiding.html
Technical the topoh module could probably support this for REGISTER messages as well, but it needs to be properly discussed and investigated for the side effects of such a change.

jumping into discussion to mention and alternative by using uac module.
There is support to do registrations to remote SIP server, so you can use the local kamailio with this module to register to foreign sip servers, and it will use only Kamailio's IP. Then your device will register to this local Kamailio and you route based on uac_reg info and location.
Enhancing topoh to support REGISTER is possible, being open source, but you need to do some C development. As pointed before, topoh target was to hide sensitive addresses between providers, not between client app/phone and its home sip server. Likely you have to take add/manage also Path headers when hiding addresses in a REGISTER request.
