I see error at log - bad_uri sip.myserver.com
try adding the port...
alias=sip.myserver.com:5060
Also, since you're behind NAT make sure you also advertise the address with advertised_address="sip.myserver.com".

P.S. I do not have any access to router, that present NAT for me.
