This is not an error. The debug messages just say that SER was unable to find the transaction in the memory and will create a new one.
If you have troubles with audio then most likely they are not caused by SER, because media streams bypass the proxy server.

Searching Google, I found that this is a known bug in Asterisk. I upgraded to 1.0.7 (Debian unstable) but bug is still present.
Peraphs, Asterisk mailing-list would be a better choice for this message but anyway can someone confirm this?
