your case might be related to the thread "[Users] OpenSER not processing "200 OK" response to CANCEL message" - Cancel is sent over and over again without considering the received reply. See: http://www.openser.org/pipermail/users/2005-October/001017.html

I just fixed this on CVS - let me know if it solved your problem.
