I haven't found any error message related to tm in the logs
you attached.
You say that if you omit the first parameter, everything
works ok? Can you try to set the first parameter to a
non-zero value?

Regarding to t_set_fr() function, i use it in several places
within my kamailio scripts without any issue, I only noticed
this behavior on this particular case.
I don't if this can be relevant but I have tested t_set_fr
function with the following parameters => t_set_fr(,2000)
and it entered on failure route after 2 seconds.
