Thanks Andres for your answer, the problem I'm having is that I'm not being able to register an extension from outside of the firewall. I would appreciate if someone could put me on the right path. I'm still not sure if I have to enable the NAT module or the DOMAIN module and add the external IP or something else, this are the SIP messages I have captured:

Depends on your configuration. The domain module just provides the functionality to check - based on the domain table - whether the domain part in From, R-URI or any pseudo-variable is present in the table.
If you want your user A to be visible and valid as A@your-domain and A@your-ip and possibly A@anything, then you should set "use_domain" in usrloc module (and probably others which you already use) to "0" to not check for the domain part at all.
