Hello,
apparently mysql doesn't like a default value for blob columns, might be because of a new version.
You have to edit msilo-create.sql and remove DEFAULT part of the statement for body column.
On the other hand, I recommend using kamailio 4.1.x, 4.0.1 is quite old.
