[OpenSER-Docs] First Clean-up Patch for Documentation

Thanks Edson, for me is ok to stick to ENTITY for now. I have to ask you 
to make an update to the docbook version in the xml files header -- in 
my example was 4.2, Henning discovered that 4.4 is the version shipped 
with Debian stable, please use that.
