this behavior has never worked for me for many reasons and i have had to
use my custom version of acc module.  now i want to get rid of that.

for example, if call comes from PSTN gw, it is not authenticated.  in
that case, i want to use a special caller UserName telling that request
came from PSTN gw rather than "n/a" or "".
