i agree that they are useful, but still should not be in the minimal
set, because they are not needed in bye message, for example, which is
matched to corresponding invite using call id and tags.  

also, as i mentioned, i don't take Called-Station-Id from To URI, but
from Request URI after i have canonized it.  for example, if user made a
call without country code and area code, To URI user part contains just
the local number. after canonization R-URI user part will contain the
full E.164 number, which is much more useful to store in
Called-Station-ID that the local number in the To URI.

README file could have an example of extra item

Called-Station-Id=$tu

which would maintain the current behavior.  the same for
Calling-Station-Id.
