[Serusers] HELP! duplicate accounting

I was able to see the accounting Logs on my radacct
directory the problem was I am having duplicate START
and single STOP.

I also see this from my radius log file.
rlm_radutmp: Login entry for NAS localhost port 5060
duplicate
