There is an issue called call-id stickiness that will cause you problems if 
you have devices behind NAT. If you don't, you should be fine with LVS.
As for your problem, you may want to post it the the LVS mailing list. I 
have never heard about this before, but it may be due to the way ldirector 
is testing "upness".  I'm quite sure it does not support an OPTIONS SIP 
message as a ping and that it will try on a lower level, which may be 
answered regardless of ser is running or not. Try turning off the network on 
the real server and see if it is detected as down. Also, do an ngrep on port 
5060 on the real server to see what you get.
