Don't know about the US, but here in Austria the simplest thing is 
terminating yourself. Gateways are quite affordable nowadays, and you 
have all the freedom you need in terms of signalling (and the gateways I 
know like Inalp/Patton, Audiocodes, Cisco *are* RFC compliant).
