modules/lcr: new feature added and old one removed

- Gateway ip_addr can now be NULL if hostname is given.  If ip_addr of
  one or more gateways in an LCR instance is NULL, all test functions
  fail on that LCR instance, because for performance reasons, no DNS
  queries are done by the test functions.
- Removed support for MI functions.
