[Serdev] maxfwd bug

i started to wonder why a certain forwarded call failed and ethereal
dump of the forwarded invite showed that its max-forwards header
contained empty body!

after some debugging i found that this was caused by a bug in
decrement_maxfwd function.  i'm very surprised that this bug had gone
unnoticed for such a long time.

patch is enclosed.  could someone please commit it to rel_0_9_0 at
least.
