[Devel] avp as a module parameter

Hi all,

When an avp is used as a module parameter, the way of passing the avp
to the module should be consistent.
Right now it is used in two different formats:
 - modparam("tm", "fr_inv_timer_avp", "i:25")
 - modparam("uac","auth_username_avp","$avp(i:11)")

We should pick one format and stick to it:
 1. $avp(avp_flag:avp_name)
 2. avp_flag:avp_name
