Did you use kamdbctl extra?  Did it create the MOHQUEUES and MOHCALLS
tables?  If so, comment out these modparams.

#modparam ("mohqueue", "db_qtable", "mqueues")
#modparam ("mohqueue", "db_ctable", "mcalls")
