Hello,

first, do not set memdbg and memlog lower than debug parameter value
because the logs become too big and a lot of useless messages. Those two
are useful when troubleshooting memory issues for Kamailio pkg/shm.

Back to the error, the issue is reflected by the error messages, the
first one is giving quite good description:

 9(2272) ERROR: db_mysql [km_my_con.c:124]: db_mysql_new_connection(): driver error: Can't create a new thread (errno 11); if you are not out of available memory, you can consult the manual for a possible OS-dependent bug
 9(2272) ERROR: <core> [db.c:318]: db_do_init2(): could not add connection to the pool 9(2272) DEBUG: <core> [mem/f_malloc.c:575]: fm_free(): fm_free(0x7f71692ac010, 0x7f7169346d60), called from core: db_id.c: free_db_id(346)
 9(2272) ERROR: usrloc [ul_mod.c:443]: child_init(): child(-1): failed to connect to database
 9(2272) ERROR: <core> [sr_module.c:917]: init_mod_child(): Error while initializing module usrloc (/usr/local/lib64/kamailio/modules/usrloc.so)

As a hint to try further to narrow down, set children parameter in
kamailio.cfg to lower value and see if it starts. If yes, for larger
value for children, you need more system memory.

Cheers,
Daniel
