Well, sure. But its applications are considerably more limited, since, 
without a database, it's pretty tough to make any "dynamic" decisions.

Could that be a domain issue? (Domain is a component of location 
resolution.)

That said, you can certainly run Kamailio in a registrar role without 
database backing, using solely the in-memory 'usrloc'.
