One more thing I might have forgotten.
Asterisk needs to responde with a 200 OK to the OPTIONS requests in order for 
Kamailio to think the dispatcher route is up and runnnig. So add an s exten in 
the asterisk context:

[incoming]
exten => s,1,NoOp()
