have you had a chance to try  just kamailio+rtpproxy? rtpproxy can 
bridge rtp packages ipv4<=>ipv6, and for sip kamailio will take care.

Also, what modules have you used in kamailio? I know some of them need 
to be reviewed for the size of ip address in database fields...
