mysql is used for location records.

Watch the network traffic (ngrep or wireshark) and see what happens with 
the REGISTER messages. Also, you can run OpenSER in debug mode and see 
log messages. One possibility is that your phone is behind NAT and your 
config does not handle NAT traversal.
