[Serusers] Problem with blind forward

Hi All!
I've been trying to enable to blind forward functinality
I did everything as it is writen in the getting started manual.
I have the following record in the database:
mysql> select * from ser.usr_preferences where attribute = 'callfwd';

Whenever i try to call the test at scsct.im.touk.pl i receive the following reply from ser:
