[Serusers] SEMS + SER howto

We will release issue 5 of the document very soon. However, this version
does not include Asterisk voicemail/IVR, but basic call features (forwarding
etc), a comparison of openser/ser and an intro on CPL. Asterisk integration
will be the focus of issue 6.
