sence without the context. Please explain your scenarios and assumptions 
before you ask questions!

No, why should it. Neither Ser nor nathelper is an endpoint, so both do not 
have to be able to create a SDP answer.
The SDP offer will be relayed to the remote side which will generate the SDP 
answer. Thus the endpoints have to support 3264 not the proxy. Except it 
would be a B2BUA. But you should know well enough that SER is not a B2BUA.

To provide MOH you need a source of the media stream, mostly called media 
server.

And finally: do NOT even think about MOH behind NAT. Several people who have a 
LOT more VoIP knowledge then you did not solved this problem yet with all 
facets. Please leave the big problems for the professionals.
