[Users] Problem in creating MySQL database

are you using a precompiled package (which distro? rpm? deb?) or do you 
complied it yourself?

the openser_mysql script can not find the openser_gen_ha1
tool. This can happen /usr/local/sbin is not included in the PATH.

You can edit the openser_mysql script and specify the correct location 
of the openser_gen_ha1 tool
