If its stateless, the u can do billing, but you will only know how loing
the call was after the call has finished and as long as you record_route
all SIp messages via yourproxy.

As for NAT yes it can, but if you are dealing with the NAT (far end
travesal) then the stream will come via you and use either rtproxy or
mediaproxy, but mediaproxy can be spread out amongst several servers,
also you could setup a STUN server which would also help to remove some
load
