[Serusers] Newbie - Installing SER and Mysql

I just installed ser-0.8.12-0.i386,
ser-mysql_ser-0.8.12-0.i386 and MYSQL 4.0.21-0.i386
witout any issues but when i run the two command below
to change mysql root passowrd and create the SER
database i get the errors below.  Please Help,

ser_mysql.sh create
MySql password for root:
creating database ser ...

found
[root at sipproxy1 sbin]#
