we have experience with Asterisk (http://www.asterisk.org). Works, and
adds a lot of other features as well (voicemail, conference brigde, you
name it). It's a soft PBX, but since it understands SIP, it can be
(mis)used as a gateway as well.

Definitely worth a look.
