mem: TLSF allocator integration in kamailio

- kamailio memory counters for statistics
- use
    MEMMNG=1 MEMDBG=0 extra_defs='-DTLSF_MALLOC=1
  to enable
- support for > 4GB memory (current limit is now 1TB)
