That seemed to do the trick. 

The problem is that when then callid changes and the registration expires, kamailio tries to delete it using the callid instead of instance.
The row is not deleted (row contains an old callid), but will be updated next time the client is saved using instance. This results in two rows containing excact the same values, except callid.

Maybe instance and reg_id should be used when a contact expires/unregisters as well?
