daniel,

i tested again with latest master and domain column now has correct
value.  however, instance column is NULL even when incoming register
contact does contain instance param:

Contact: <sip:0x9b6ece8 at 188.67.246.168:33623;transport=tcp>;expires=600;+sip.instance="<urn:uuid:0ec1fda0-02d6-4188-8c44-e15d1fb0d393>".

if the idea is to store instance value to the column, there is still a
bug left.
