[Patch] pa module's free_presentity() doesn't free extension elements

This is not an issue in normal operation as extension elements are freed correctly
when they expire or are deleted explicitly.

It is however an issue when you're searching for memory leaks.

The patch applies to both 2.0 and HEAD.
