Summary: lcr fails when used first time in failure route

Initial Comment:
For the case of call forwarding on no answer or busy, lcr fails.

If the user has redirection to PSTN for No Answer or Busy, so that lcr has to be applied on failure route it throws the error message:

"No ruri_user AVP"

It is searching for ruri_user_avp, which was not set as the function was not called in "route" before.

Is this intentional for some reasons or it is a bug?

It is now possible in Kamailio 1.5 to call load_gws() and next_gw()
functions also from FAILURE_ROUTE.

The solution is not perfect yet, because it is not currently possible to
determine in REQUEST_ROUTE if t_relay() has already been called, i.e., if
next() should rewrite Request URI or append a new branch.

load_gws() forced to route usage only, moved this as feature request to
have in mind for a review and possibility to enable it in other route
types.
