Summary: Improve Doxygen docs

Initial Comment:
This patch improves doxygen documentation by adding doxygen markers to a lot of comments, adding file declarations and also adding a few pages that will show up under "related pages". These are existing docs in the file, but wasn't properly marked up.

The patch touches many files, but doesn't change any source code. Only comments are affected.
