acc_rad_request accs username, which contradicts README:

   The fixed minimal accouting information is:

     * Request Method name
     * From header URI
     * From header TAG parameter
     * To header URI
     * To header TAG parameter
     * Cseq number
     * Call-Id
     * 3-digit Status code from final reply
     * Reason phrase from final reply
     * Time stamp when transaction was completed

i suggest not to include username in minimal list, since it may be taken
from other places than p-auth header or from header (for example from
p-preferred-identity header).  i also suggest to drop "to header uri"
from the minimal list, since to uri makes no sense anyway.  further, i
don't see need to include cseq either.
