Summary: OSP module flips caller/callee in CDR

Initial Comment:
When a proxy is configured as originate and terminate 
at the same time (src gateway->proxy->proxy->proxy-
>dst gateway), if the dst gateway release the call 
first, the OSP module flips the caller and callee in 
the CDR.

This bug has been fixed by using append_fromtag approach. 
The changes are committed into CVS head on 7/24/06.
