Make your mysql.so really exists in that directory (/usr/local/lib/ser/modules/mysql.so). It seems that you didn't make and install SER with MySQL support. To do so follow the instructions shown in www.iptel.org/ser in the install document, or in www.onsip.org, in the getting started document, where it explains step by step how to make and install SER including MySQL modules.
