Thanks to clean up that understanding block :)

I still have a (little?) issue in my Installation. I installed the Kamailio with standard configuration and enabled "WITH_NAT".

If I now register two clients (both Zoiper) they try to do the RTP P2P to each other. But I want to process the RTP on the Server. What Parameter do I have to change?

Yes, Kamailio is a SIP proxy and does not deal with RTP, so rptproxy is the tool to handle media traffic in your case. 

rtpproxy is not the only choice, you also can check rtpengine. https://github.com/sipwise/rtpengine
