If I'm not mistaken, Refer-To is a name-address pair, so the best way to access the user part of the URI is:
$(hdr(Refer-To){nameaddr.uri}{uri.user})
i.e.
if($(hdr(Refer-To){nameaddr.uri}{uri.user}) =~ "^011")
