Hi,

I would setup an additional Kamailio only for the internal stuff that does what you need.

You can combine it with your external and point all asterisk to the internal which makes the routing decision asterisk to asterisk or asterisk to external.

To put everything in one Kamailio is possible but of the cost of complexity.
