I have just pushed the feature to a new branch here: https://github.com/kamailio/kamailio/tree/cchance/ht-dmq-sync.

The specific commit is here: https://github.com/kamailio/kamailio/commit/57f87fcb3a973735681dba5c9e9484c5664db3aa.

Feel free to test it out - you'll need to set the modparam to enable it...

modparam("htable", "dmq_init_sync", 1)
